Chronic Hives
Root-Cause Care for Chronic Hives
Chronic hives are a sign that your mast cells are on high alert, and the reasons for that are usually found inside the body.
The Basics
What hives actually are
Hives are raised, itchy welts that can appear anywhere on the body, move around, and fade within hours, only to show up somewhere else. They happen when immune cells in the skin called mast cells release histamine and other chemicals that make small blood vessels leak fluid into the skin. When hives show up most days for six weeks or longer, they are considered chronic.
Some people also get deeper swelling of the lips, eyelids, hands, or feet, called angioedema, or welts that appear wherever the skin is scratched or pressed.
If you ever have swelling of the tongue or throat, or trouble breathing or swallowing, that is an emergency, and you should call 911.
Why hives keep coming back
Most people with chronic hives are treated with antihistamines, often at high doses, sometimes with short courses of steroids, and for some, an injectable biologic. These can be very helpful for controlling symptoms. But many people feel foggy or tired on antihistamines, and most chronic hives are labeled "idiopathic," which simply means no cause was found. That label is often where the search stops, and it is where mine starts.
One example of what that search can turn up: chronic hives are significantly more common in people with autoimmune thyroid disease (Tienforti, 2022), which is why the thyroid is part of the workup.
What I Look For
What is often driving it
Chronic hives rarely have a single cause. These are the patterns I look for most often, some that set off a flare and some deeper inside the body.
What Sets Off a Flare
Mast cells on high alert
Mast cells are designed to respond to threats, but in chronic hives they become overly sensitive and release histamine in response to things that shouldn't bother them, including food, heat, pressure, stress, and infections.
Histamine overload
Your body breaks down histamine from food with an enzyme called DAO, which is made in the lining of the gut. When the gut lining is damaged or certain medications block that enzyme, histamine builds up. High-histamine foods like aged cheese, cured meats, wine, fermented foods, and leftovers can then push you over the edge.
Food and additive triggers
True food allergies, most often to dairy, eggs, peanuts, wheat, soy, fish, shellfish, or tree nuts, can cause hives. So can food additives like MSG and preservatives, and natural compounds called salicylates, which are found in many fruits, vegetables, and spices as well as aspirin and some personal care products.
From the Inside
Gut imbalance and leaky gut
A more permeable gut lining lets food proteins reach the immune system, where they can set off reactions. An imbalance in gut bacteria also changes how histamine is handled. People with histamine intolerance tend to have a different mix of gut bacteria (Schink, 2018), and in SIBO, the overgrowth is often driven by bacteria like Klebsiella, which produce histamine (Leite, 2024).
Hidden infections
H. pylori, a stomach bacterium, has been linked to chronic hives, and treating it can help (Kim, 2019). Candida can also activate mast cells directly (Lopes, 2015), and other chronic infections can keep them on alert.
Heavy metals, mold, and vitamin D
Heavy metals such as mercury can change how mast cells behave (Suzuki, 2011), and mold is another exposure I look for. Low vitamin D can also make the immune system more reactive, and in one study, adding vitamin D improved symptoms and quality of life for people with chronic hives (Nabavizadeh, 2023).

C: Cut inflammatory foods. An elimination diet, often with a low-histamine phase, followed by careful reintroduction to find your specific triggers, including additives.
L: Lower toxic burden. Looking for mold and other environmental exposures, and creating a less reactive home and personal care routine.
E: Eliminate gut pathogens. Testing for and treating H. pylori, SIBO, Candida, and other gut imbalances that keep mast cells activated.
A: Absorb key nutrients. Restoring vitamin D and using nutrients and natural compounds that help stabilize mast cells.
R: Rebuild and Restore. Repairing the gut lining so it can make enough of the enzyme that clears histamine, and calming the stress response, since stress is one of the things that sets mast cells off.
What to expect
You don't have to stop your antihistamines to work with me. We usually keep them in place while we look for what is driving your hives, then step them down gradually as your skin calms down. Many people notice they need less medication as their gut heals and their triggers come into focus.
